Coaching Consent Form for Parents and Carers

Please read this form carefully, complete all sections, and sign at the end. Coaching for anyone under 18 begins only after a signed consent form has been received.

  • Coach / Organisation: Unmask Your Potential
  • Purpose of coaching: Personal development, reflective practice, confidence building, and leadership skills tailored for young people.
  • Typical session length and frequency: (e.g. 45 - 60 minutes weekly / fortnightly/monthly)
  • Location: (in-person address or online platform)
  • General themes likely to be covered: self-awareness; goal setting; communication; resilience; emotional regulation; creative reflection.

Insurance

  • Public liability insurance: Coach/organisation holds public liability insurance to cover incidents or injury occurring during in-person sessions.
  • Professional indemnity insurance: Coach/organisation holds professional indemnity insurance covering the professional advice and coaching services provided.
  • Coverage during activities: Insurance covers usual coaching activities, on-site sessions at agreed venues, and coach-supervised off-site activities included in the programme.
  • Policy details available on request: Insurer name, policy numbers, coverage limits, and policy expiry dates can be provided to parents/carers on request.
  • Limitations: Insurance does not replace statutory safeguarding responsibilities; parents/carers should inform the coach of any particular concerns that may affect safety during sessions.

Consent

  • I give permission for my child named above to take part in coaching sessions with the coach named above.
  • I understand the stated purpose, general themes, typical session length, frequency, and location of sessions.
  • I understand that coaching is not a substitute for mental health or medical services and I will seek professional help where appropriate.
  • I understand that I will be informed of session dates, times, and the general themes to be covered.
  • I consent to receiving routine administrative communications about sessions by phone, text, or email.
  • I understand that the coach may provide non-detailed progress summaries on request that respect the young person’s dignity and autonomy.
  • Confidentiality will be respected. Information shared by the young person in coaching sessions will be treated as confidential and will not be shared with others without consent.
  • Limits to confidentiality. I understand that confidentiality does not apply where there is a safeguarding concern, a serious risk to the young person or others, or where disclosure is required by law. In such cases the coach will share information with appropriate adults or authorities.
  • Proportionate sharing. Only the minimum necessary information will be disclosed to protect the young person. Any disclosures and the reasons for them will be recorded.
